Home and Community Assistance
For clients seeking to maintain their independence while staying active in their community. This program includes help with household tasks, meal preparation, shopping, and transportation to community events or appointments.
Insurance/Funding We Accept
Health and Wellness Waiver
Managed by one of the five Case Management Organizations. Usually for children and adults with complex medical needs who require specialized support to remain safely at home.
Pathways Waiver
Managed by United, Humana and Anthem for individuals aged 60 and older with disabilities needing help with daily tasks to maintain independence.
Traumatic Brain Injury Waiver
Provides support for individuals of any age who have experienced a traumatic brain injury and require specialized home care.
CHOICE
A state-funded program helping seniors and people with disabilities who don't qualify for Medicaid but still need in-home assistance.
VA Funding

Veterans and their surviving spouses may qualify for home care benefits to help with daily living through VA-sponsored programs.
Private Pay

In-Home Assessment
We will visit you at home to conduct a comprehensive assessment. This allows us to understand your daily routine, medical history, and personal preferences to build a personalized care plan tailored specifically for you.

Care Plan Review
Based on our assessment, we'll present a detailed care plan. You’ll have the opportunity to review and refine the proposed services, schedule, and frequency to ensure everything aligns with your lifestyle and expectations.

Caregiver Matching
We match you with a dedicated caregiver from our professional team who best fits your needs and personality. During the onboarding, we review policies, sign documentation, and ensure a comfortable start to your care experience.

Begin Your Services
Once the care plan is finalized, your Boost at Home caregiver begins providing support in the comfort of your home. We maintain consistent communication to ensure your ongoing satisfaction and well-being.
