TB Testing Locations

Chest X-Ray (only for caregivers with prior positive TB test)

Caregivers who have previously tested positive and cannot do a skin test will need a chest X-ray. Options include:

  • Any urgent care with imaging (Community Health Network, Franciscan Health, Ascension)

  • Primary care physician referral

Background Check Information

Disqualifying Convictions (IC 16-27-2-5)

Under Indiana Act 350 (2016), any person operating a personal services agency must obtain either a national criminal history background check (FBI fingerprint-based, available through the Indiana State Police INkless program) or an expanded criminal history check for each employee.
 

Under current Indiana law, a PSA may not employ someone to provide in-home services if their background check reveals any of the following:


Permanent bars (no time limit):

  • Any sex crime (IC 35-42-4)

  • Murder (IC 35-42-1-1)

  • Voluntary manslaughter (IC 35-42-1-3)

  • Exploitation of an endangered adult (IC 35-46-1-12)

  • Failure to report battery, neglect, or exploitation of an endangered adult (IC 35-46-1-13)


Time-limited bars:

  • Theft, conversion, or receiving stolen property — felony within the last 10 years, or misdemeanor within the last 5 years

  • Identity deception — felony within the last 10 years, or misdemeanor within the last 5 years

  • Fraud — felony within the last 10 years, or misdemeanor within the last 5 years

  • Any crime of violence (as defined in IC 35-50-1-2) within the last 10 years

  • Felony battery within the last 5 years

  • Any felony relating to controlled substances within the last 5 years

    Registry-based bar:

  • The person has a finding entered into the state nurse aide registry for abuse, neglect, mistreatment of a patient, or misappropriation of a patient's property
